Choose one of the options, either credit card or checking/savings account to set up your AutoPay.

Credit Card:
To have payments charged to your credit card, fill in the application including your credit card account number, expiration date, and signature.

Bank Account :
To have payments deducted from your savings or checking account, fill in the application including your bank account number, routing number and send us a blank check marked VOID.

Mail It In :
Send in the completed application, with a voided check using the Bank Account option in a stamped envelope to: CATV Service, Inc. 115 Mill St. Danville, PA 17821 or bring it to your local CATV Customer Service Center.
